The Official Osteopathic Physician Profile Report features comprehensive,
up-to-date, primary-source information from the AOA. The report includes
AOA accredited/approved pre- and postdoctoral education and training
dates of attendance and completion
All state licenses held with expiration dates
Osteopathic Specialty Board Certification(s) expiration dates and participation in OCC
Certification by member board(s) of the American Board of Medical
Specialties® (ABMS®) and participation in MOC
AOA accredited CME status
State and federal sanction alerts
PRIMARY SOURCE AUTHORITY
The Report, used in combination with established credentialing
procedures, meets certain primary source requirements for osteopathic medical
education, osteopathic postdoctoral training and osteopathic specialty
board certification for the following accrediting agencies: the AOA Healthcare
Facilities Accreditation Program; TJC; NCQA; URAC; and the NAIC.

These fees do not apply to CVO's or AOA physician members.
AOA physician members can send their profiles free of charge to State
Licensing Boards. Continue to the registration

Unused account balances will be refunded upon written request on company letterhead.
Charges for profiles are not refundable.
A replacement profile report will be issued for mistaken orders or profiles requiring correction.
